Refills will be handled based on standard clinical criteria for refills
 
Denials:
Refills may be denied if further clincical evalution is necessary, patient has not had a follow-up appointment or in the professional opinion of the Physician a refill is not necessary, or there has been non-compliance with recommendations. Patients may also be referred back to the Primary Care Physician for follow-up and refill requests. Many insurance plans will limit/restrict the amount, type, or duration of thearpy, and/or require re-evaluation or referral back to Primary Care Physicians.

Substitutions:
Generic substitutions and/or changes to eqivalent thearpy may be part of your insurace contract for pharmacy benefits -- you should contact your insurance company or benefits administrator for details.
