New Patients

Referrals:
Patients may make an appointment on their own or be referred by another Physician. Many insurance's require an authorized referral from the primary care Physician for a specialist's consultation.
 
Appointments:
Appointments may be made by calling the office between 8:30am and 5:00pm. You can also request an online appointment. Those wishing to call for an appointment are advised to do the following:
   1. Obtain proper referral from referring Doctor
   2. Confirm time and location of appointment.
   3. Obtain copies of labs, x-rays, and other data that helps in evaluation.
   4. Provide copy of insurance card or forms.
   5. Complete patient registration and evaluation forms prior to office visit. These forms can be downloaded, printed, and filled out beforehand for your convenience.
   - If appointments are cancelled the office can be notified for rescheduling if necessary.
 
This process is for a non-emergency only. In the event of an emergency contact your Physician and/or go to the nearest Emergency Room.
